LIFE's Case Management services help seniors and adults with disabilities get the right community supports in place so they can remain independent, safe, and surrounded by the comforts of home.

 

 Case Management Services
 

Case management professionals work with older adults and their families to:

  • Identify current and future needs
  • Develop a comprehensive plan to put services in place that meet the identified needs
  • Maintain on-going contact to make sure needs continue to be met 
  • Provide information about community aging services
  • Provide in-home counseling to homebound individuals

ADvantage Medicaid Waiver Program

Independent Case Management
