Also known as the Oklahoma Home- and Community-Based Medicaid Waiver Program, ADvantage provides in-home assistance to avoid or delay nursing home placement. Case management, services may include personal care; homemaker/chore service; durable medical equipment; incontinence, ostomy, and diabetic supplies; nutritional supplements; respite care; adult day services; and in-home skilled nursing care. Applicants must require nursing home-level of care and be medically and financially approved for the program. Contact local DHS office or your DHS case worker to determine eligibility. See list of DHS locations at MEDICAID INFORMATION.

Long Term Care Planning

A collaboration between Centers for Medicare and Medicaid Services, Agency on Aging, and the office of Assistant Secretary for Planning and Evaluation.  Government Web site designed to assist with planning ahead for long-term care needs. It includes a variety of user friendly on-line tools. You may also download a free long-term care planning kit or call (866) 752-6582 to order.

On-line database containing nursing home and long term care facility information, including quality ratings and measures, health and fire safety inspections, staffing, payment options, and alternatives. All Medicare- and Medicaid-certified nursing homes are included.
