Job Title: Program Assistant (CNA) - FT & PRN
Position Summary: This position provides therapeutic life enriching activities, interactions, and social support for frail older adults or those with profound disabilities or development delays, encouraging independence in life skills as much as possible, under direction of Adult Day Services managers and coordinators. This includes personal care assistance, health care and nutritional monitoring under the direction of nursing staff and Dietary Consultant. Facilitation of a positive social atmosphere is required of this position.
Education And Experience: C.N.A. required; minimum of 1 year experience as a CNA required

Job Title: PRN Site Nurse (LPN)
Position Summary: Provides coverage for LPN staff at any one of three sites as needed.  Provides health related services including monitoring and education to Adult Day Services participants and provides caregiver/family communication concerning same.  Provides nursing care and administration of medications according to physician.  Position is supervised by the Adult Day Services Site Manager at each location.
Education And Experience: Graduation from an accredited school of practical nursing and a minimum of two years experience in geriatric nursing.

Job Title: On-Call Receptionist
Position Summary:

Greets participants, families, and caregivers, escorting them in and out of the Adult Day Service program for safety purposes.  Accurately records attendance hours, keeping logs for audits.  Generates Participant Log weekly.

Skills:  Excellent verbal communication skills with older adults and caregivers.  Good interpersonal skills to facilitate working relationships.  Accurate writing skills; experience working with the memory and cognitive impaired; positive, professional attitude and appearance; and familiarity with computer data entry.

This position will cover our current reception staff on an as-needed basis and must be willing to work at at least two of our three Adult Day Service sites.  Hours will vary, but shifts are daytime hours, Monday through Friday.

Education And Experience: High school graduate or equivalent, with two years of geriatric experience preferred.
