Is there a minimum age requirement? LIFE serves adults age 18 and over. Younger adults we serve may be stroke survivors, have a traumatic brain injury (TBI) or a developmental disability.
What kind of financial assistance is available? We work closely with families to help them access financial assistance from the Oklahoma Department of Human Services (OKDHS), the Veteran’s Administration, the ADvantage Medicaid Waiver Program, the Oxford Respite Grant, and the OASIS Respite Voucher program.
How is the program staffed? We maintain an appropriate staff to participant ratio according to CARF and Oklahoma State Department of Health standards. Professional staff include: an LPN at each site, Certified Therapeutic Recreation Specialist (CTRS), activity coordinators, Certified Occupational Therapy Assistant (COTA), Bachelor’s-prepared site managers and Family Assistance Coordinators, Certified Nurse Aides (CNA) and Medication Administration Technicians (MAT). Each staff member is a CAN who has completed Dementia Specific Training and is certified in American Red Cross First Air, CPR and AED.
What are minimum requirements for participation? Adults with a wide range of disabilities routines attend and benefit from Adult Day Services. Generally, participants must be partial weight bearing, exhibit appropriate behavior that does not put others at risk, and not require prolonged bed rest.
What acuity level do you accommodate? Working with the individual’s private physician, LIFE’s nursing services regularly accommodate respiratory treatments, tube feedings, injections, incontinence care and simple wound care. We frequently provide care for individuals receiving hospice services as long as prolonged bed rest is not required.
How do you handle medications? Medications, prescriptions and over-the-counter, are administered by licensed or certified staff as prescribed and based on physicians’ orders.
Do you have special accommodations or services for individuals with Alzheimer’s disease or other dementia? LIFE has nearly 30 years of experience providing care and services to individuals with memory loss. Each center is designed to enhance the safety, comfort and independence of participants with dementia. Security features include magnetic locks and keypad entries to prevent elopement. Specially designed dementia activities and supportive care are provided by an experienced and well-trained staff, each having completed Dementia Specific Training. Family can rest easy knowing their loved is safe, having a good time, and receiving appropriate care and attention.
Do you offer therapy services? Physical therapy services (physical, speech and occupational therapy) are available with physician’s orders at all three sites through an independent Physical Therapist, and can be billed to Medicare or private insurance.
What kinds of personal care services do you provide? We provide assisted showers, incontinence care, foot care and the services of a fully-licensed beauty salon.
Are meals included? What about special diets? A nutritious breakfast, lunch and two snacks are served daily. Meals are prepared on-site. Special diets including renal, modified diabetic, no added salt, and soft and pureed are available with physician’s orders.
Do you take "drop-ins"? To maintain our high standard of care and to ensure maximum benefits from the program, drop-ins are not recommended. We have found that regular participation (ideally at least two days per week) is most advantageous for both the participant and the caregiver.
What about transportation? The Family Assistance Coordinators help participants access transportation services through MTTA/Tulsa Transit. The Lift program in Tulsa and Route 508 in Broken Arrow provide transportation to the centers for families unable to provide transportation themselves.
How does a person enroll or get more information? For additional information, to arrange for a tour or to begin the enrollment process, please call DeLana Duvall at (918) 664-9000, ext. 236.
